Neuromodulator injection therapy (specifically Onabotulinumtoxin A) has been used to treat chronic migraines for over 2 decades with several clinical trials illustrating it’s efficacy in reducing the frequency, severity and intensity of headaches. These injections are thought to block the release of pain receptor chemicals (neurotransmitters) targeting the brain’s outer sheaths (meninges). Clinical trials showed 40-50% of patients had a greater than 50% reduction in the number of headache days per month and it received FDA approval for this very use in 2010.

Headaches are the most frequent neurological complaint globally. Chronic migraines affect 12% of the world’s population and are defined as headaches (4-72 hours long) for 15 days in a month for 3 months, 8 of those being migrainous. These chronic headaches may or may not have accompanying neurological symptoms/signs called auras.

Neuromodulator injection therapy is an ideal option for chronic migraine sufferers who’s headaches may still be present despite other preventative medication, or in those who cannot tolerate the plethora of side-effects from prescription medication.

Chronic migraines are a PMB (prescribed minimum benefit) and most medical aids cover (partially or wholly) for this specialised treatment. Neuromodulator injection therapy for chronic migraines are very safe & relatively painless when administered by an experienced, licensed professional. Dr. Djan applies a customised approach giving multiple neuromodulator injections over the head and neck, which cover the common trigger points of chronic migraines. Injections may be repeated in 4 months if it halves the
frequency of the migrainous headaches. The migraine relieving effects of the injections may take a few weeks to become apparent and everyone’s response rate may differ. Some patients find they can discontinue injections without frequent migraines returning. Others need regular treatments to keep migraines under control.

Some transient and rare effects from neuromodulator injection therapy injections include mild discomfort at the injection sites, droopy eyelids (ptosis) or dry eye syndrome.

Neuromodulator injection therapy is an extremely effective treatment for bruxism, a condition where a person unconsciously grinds or clenches their teeth, especially during sleep. While mouthguards and behavioural therapy are efficient options in this condition, they do not always provide maxim relief. These injections causes transient relaxation of the muscles responsible for clenching, primarily the masseter muscles (large jaw muscles). This often reduces the intensity of grinding, mitigates associated jaw pain/

spasms and prevents further damage to teeth.

Neuromodulator injection therapy for bruxism is a very safe and relatively painless procedure when administered by an experienced, licensed professional. Dr. Djan injects small doses into specific areas of the jaw muscles and the muscle-clenching relief becomes apparent 1-2 weeks later. The therapeutic effect generally lasts 4 to 6 months, after which the treatment is repeated if needed.
